Consider:

1. Upon receiving the form, the doctor's assistant will contact you to indicate the available date and time and will send the instructions.


2. In the box "topic to be covered in the orientation" describe the reason for the orientation to determine if it is possible to do it virtually.


3. Guidance as the name implies is to resolve questions or concerns. This counseling service is NOT a substitute for personal medical consultations, it is NOT formulated for patients and does NOT generate medical disabilities.


4. On orientation day, please have on hand: medical history, test results and annotated questions.
