1. Upon receiving the form completed, the doctor's assistant will contact you to indicate the available date, time and instructions.

3. Virtual guidance, as the name implies is to resolve questions or concerns. This counseling service is NOT a substitute for personal medical consultations, medications will NOT be prescribed virtually, and NO medical excuse notes will be processed this way.
4. On the day of the virtual meeting, please have on hand: medical history, test results, and the written questions.
