Consider:

1. Upon receiving the form completed, the doctor's assistant will contact you to indicate the available date, time and instructions.


2. In the field "topic to be addressed in the guidance", please describe the reason for the guidance to verify if it is possible to perform virtually.


3. Virtual guidance is NOT a substitute for neuropsychology consultations or a psychotherapeutic process. Both can be performed virtually but their objectives are different. In the guidance, answers will be given to specific questions that can be answered in a concise way via email or other answer option, and will not last more than 15 minutes.


4. Child patient: the history of Pediatric Neuropediatrician and/or Child Psychiatry if you are a child plus any head examination that has been carried out such as a CT electroencephalogram or MRI of the brain.


5. Adult patient: history of Neurology and/or Psychiatry plus any head examination they gave you, such as a CT EEG or MRI of the brain
