Sin embargo, a veces es suficiente en el tratamiento de parejas infértiles una pequeña ayuda como es el caso de las relaciones sexuales dirigidas o programadas, este procedimiento sencillo puede ser la primera y única solución en casos en que no se logra tener un embarazo. </p>
 
<p>Las relaciones sexuales dirigidas son el primer escalón terapéutico en fertilidad, habitualmente sólo se lleva a cabo una evaluación de la pareja con un estudio básico del estado de las trompas y niveles hormonales. </p>
 
<p>Para obtener los mejores resultados, es importante que la mujer tenga menos de 35 años, las trompas de Falopio de la mujer sean permeables y que el semen de su pareja se encuentre dentro de parámetros normales. </p>
 
<h2>INDICACIONES:</h2>

                        <h2>¿Cómo se realizan las relaciones sexuales dirigidas (RSD)?</h2> 
                      <p>  Las relaciones sexuales dirigidas, tiene como objetivo concentrar las relaciones sexuales de la pareja durante los días más fértiles de la mujer. Para ello, se requiere un monitoreo ecográfico ovulatorio para definir el momento de la ovulación: Esta técnica se puede seguir de tres formas; con el ciclo fisiológico normal, con el desencadenamiento de la ovulación o bien optar por un ciclo con estimulación ovárica, a continuación, se explicare cada uno de ellos. </p>
 
<h2>Ciclo natural y ovulación espontánea:</h2> 
 
 <p> <strong>PASO UNO:</strong> En el caso que la mujer presente ciclos menstruales regulares, es de esperar que en cada ciclo crezca un folículo en uno de los dos ovarios (Mayor de 18mm). Este crecimiento se comprueba mediante monitoreo ecográfico y del mismo modo se puede predecir el día de la ovulación.  </p>
 
 <p> <strong>PASO 2: </strong>Cuando se establezca el momento de la ovulación, a partir de esta fecha se ordenará a la pareja mantener relaciones sexuales. </p>
 
 <p> <strong>PASO 3:</strong> Se realizará un nuevo control ecográfico para confirmar que ocurrió la ovulación y si esta se confirma, se ordenara la aplicación de progesterona para ayudar a mantener el embarazo. </p>
 
 <p> <strong>PASO 4:  </strong>Se ordenará la realización de una prueba de embarazo. </p>
 
 

 <h2>Ciclo natural e inducción de la ovulación: </h2>
 
 <p><strong>PASO UNO: </strong>De igual forma, en el caso que la mujer presente ciclos menstruales regulares, es de esperar que en cada ciclo crezca un folículo en uno de los dos ovarios mediante monitoreo folicular. </p>

 <p><strong>PASO CUATRO: </strong>Se realizará un nuevo control ecográfico para confirmar que ocurrió la ovulación y si esta se confirma, se ordenara la aplicación de progesterona para ayudar a mantener el embarazo. </p>
 
 <p><strong>PASO CINCO:  </strong>Se ordenará la realización de una prueba de embarazo. </p>
 
  
<h2>Ciclo con estimulación ovárica: </h2>
    <p>Existen casos especiales, en que se requiere realizar una ligera estimulación de los ovarios para ayudar al crecimiento de los folículos. </p>
 
 
<p><strong>PASO UNO:  </strong>La estimulación ovárica se inicia coincidiendo con los primeros días de la menstruación y habitualmente comienza entre el día 3 y 5 del ciclo menstrual. El tipo de fármaco a utilizar y su dosis se establece a partir de las condiciones propias de cada paciente. 
 </p>
<p><strong>PASO DOS: </strong>Una vez el folículo adquiere el tamaño necesario (18-20mm), se administra una medicación específica (hCG) para asegurar que ocurra la ovulación.  </p>

<h2>¿Cuándo se debe cancelar un ciclo de Relaciones Sexuales Dirigidas):</h2> 
                        
                        <p>Si como consecuencia de la administración de fármacos se produce el crecimiento de más de tres folículos de un tamaño considerable (alrededor de 16mm o más), es necesario cancelar el ciclo para evitar las probabilidades de gestación múltiple (tres o más bebes). Por el mismo motivo, es recomendable que la pareja no mantenga relaciones durante ese ciclo. </p>

<h2>TECNICAS DE REPRODUCCIÓN ASISTIDA DE ALTA COMPLEJIDAD</h2>
<br/><br/>
<h2>Fecundación In Vitro (FIV): </h2>

                       <p>La fecundación in vitro, FIV o (IVF en inglés) es la técnica convencional en la que se persigue la generación de un embrión humano mediante la fecundación de un ovocito por un espermatozoide maduro, en medios de cultivo (fuera del cuerpo de la madre), para posteriormente transferir al útero materno y que este evolucione hasta conseguir un embarazo. La FIV es el principal tratamiento para la infertilidad cuando otros métodos de reproducción asistida no han tenido éxito.  </p> 
 
<h3>¿Cuándo está indicado la fertilización in vitro (FIV)? </h3>
                        <p> Las indicaciones principales indicaciones de la FIV son: </p>
                        <ul>
                            
                            <li>Obstrucción de las trompas</li> 
                            <li>Falla ovárica prematura </li> 
                                <li> Endometriosis severa, cuando fallaron otros tratamientos previos </li> 
                                    <li> Factor masculino moderado </li> 
                                        <li>Infertilidad inexplicable </li> 
                                            <li>Inseminaciones intrauterinas fallidas </li> 
                                                <li>Diagnóstico genético preimplantacional (DGP)</li> 
                        </ul>

                        <h2>Microinyección Intracitoplasmática de Espermatozoides (ICSI)
  </h2>

                        <p>La fecundación de los óvulos puede llevarse a cabo también por otra técnica denominada Inyección Intracitoplasmática de Espermatozoides (ICSI). En esta técnica sólo un espermatozoide es inyectado directamente en el citoplasma, atravesando la zona pelúcida.  </p>
 
<p>El ICSI se usa principalmente en aquellos casos en los que hay un factor masculino de infertilidad, como una baja concentración de espermatozoides en el esperma (azoospermia, oligozoospermia), problemas de movilidad en los espermatozoides, antecedente de vasectomía, enfermedad infecciosa (VIH, hepatitis,), infertilidad de causa inmunitaria, o porque resulta imposible conseguir una eyaculación en condiciones normales.</p>
 
 
<h3>¿Cómo se realiza la FIV o ICSI? </h3>
                        <p> Fecundación in vitro (FIV) o (ICSI) requieren de similares pasos, solo difiere al momento de la fecundación en el laboratorio. Para entender mejor cómo funciona la FIV/ICSI a continuación describimos sus pasos: </p>
 
 <ul>
<li><strong>PASO UNO: </strong>Estimulación ovárica En esta fase se administra medicación hormonal mediante inyecciones subcutáneas a la paciente. Dicha medicación estimulará al ovario a que produzca el mayor número posible de ovocitos de buena calidad. El tipo de fármaco a utilizar y su dosis se establece a partir de las condiciones propias de cada paciente. </li>
 
<li><strong>PASO DOS: </strong>Monitoreo folicular e Inducción de la maduración ovocitaría. Durante el tiempo de estimulación se controlará el crecimiento de los folículos mediante monitoreo ecográfico para establecer cuándo están los folículos en un tamaño (mayor de 18mm), y así administrar una medicación específica (hCG) para asegurar que los ovocitos completen la maduración, a su vez, se administrara otra medicación que evite la ovulación hasta el día de la extracción de los folículos. </li>
 
<li><strong>PASO TRES:</strong> Aspiración folicular. 36 horas después de la medicación para inducir la maduración ovo citaría, se realiza la aspiración folicular.  Mediante una ecografía vaginal y bajo sedación, se realiza una punción de los ovarios que permite extraer el líquido que contienen los folículos y estos posteriormente serán analizado en el laboratorio para encontrar los óvulos. 
 <br />
Tras la intervención, la paciente permanecerá una o dos horas en reposo y después podrá volver a su domicilio. El resto del día no requerirá hacer reposo, pero sí evitar sobreesfuerzos. </li>
 
 
<li><strong>PASO CUATRO: </strong>Fertilización FIV/ICSI Una vez se disponen de los ovocitos, tras la realización de la punción folicular, y de los espermatozoides, bien por parte de la pareja masculina o bien de un donante, se procederá a la inseminación de los óvulos. Los embriones resultantes tras la inseminación son observados en laboratorio día tras día y serán clasificados según su morfología y capacidad de división. </li>
 
<li><strong>PASO CINCO: </strong>Transferencia Embrionaria Una vez se obtengan los embriones se procederá a realizar la transferencia dentro del útero, la cual puede programarse en el mismo ciclo, o en algunos casos se requiere la criopreservación de los embriones y posteriormente realizar la transferencia en otro ciclo. </li>
 
<li><strong>PASO CINCO: </strong>Prueba de embarazo Se realiza la prueba de embarazo cuantitativa (BHCG) en sangre. </li>
 
<li><strong>PASO SEIS: </strong>Vitrificación de embriones restantes con el fin de poder ser transferidos en un ciclo posterior se procede en el laboratorio a la congelación o vitrificación de embriones restantes no utilizados.</li></ul>

<h2>In Vitro Fertilization (IVF):</h2>

<p>In vitro fertilization, IVF (or FIV in Spanish), is the conventional technique aimed at creating a human embryo by fertilizing an oocyte with a mature sperm in a culture medium (outside the mother's body), and later transferring it into the uterus to develop into a pregnancy. IVF is the primary treatment for infertility when other assisted reproductive methods have not been successful.</p> 

<h3>When is IVF indicated?</h3>
<p>The main indications for IVF are:</p>

    <li>Tubal obstruction</li> 
    <li>Premature ovarian failure</li>

    <li>Moderate male factor infertility</li>

<h2>Intracytoplasmic Sperm Injection (ICSI)</h2>

<p>Fertilization of oocytes can also be performed using another technique called Intracytoplasmic Sperm Injection (ICSI). In this technique, a single sperm is directly injected into the cytoplasm, penetrating the zona pellucida.</p>

<p>ICSI is primarily used in cases of male infertility factors, such as low sperm concentration (azoospermia, oligozoospermia), poor sperm motility, history of vasectomy, infectious disease (HIV, hepatitis), immunological infertility, or when ejaculation under normal conditions is not possible.</p>

<h3>How is IVF or ICSI performed?</h3>
<p>In vitro fertilization (IVF) or ICSI require similar steps, differing only at the moment of fertilization in the laboratory. To better understand how IVF/ICSI works, here are the steps described:</p>

<ul>
<li><strong>STEP ONE:</strong> Ovarian stimulation. In this phase, hormonal medication is administered via subcutaneous injections to stimulate the ovaries to produce as many good-quality oocytes as possible. The type and dosage of medication are determined based on the patient’s specific condition.</li>

<li><strong>STEP TWO:</strong> Follicular monitoring and induction of oocyte maturation. During stimulation, follicular growth is monitored via ultrasound to determine when the follicles reach a suitable size (greater than 18mm), at which point a specific medication (hCG) is administered to ensure oocyte maturation. Additionally, another medication is given to prevent ovulation until the day of follicle retrieval.</li>

<li><strong>STEP THREE:</strong> Follicular aspiration. 36 hours after administering the maturation trigger, follicular aspiration is performed. Under vaginal ultrasound guidance and sedation, the ovaries are punctured to extract the fluid from the follicles, which is then analyzed in the lab to retrieve the oocytes.
<br/>
After the procedure, the patient remains at rest for one or two hours and can then return home. No strict rest is needed afterward, but strenuous activities should be avoided.</li>

<li><strong>STEP FOUR:</strong> IVF/ICSI fertilization. Once the oocytes are retrieved and sperm is available (from a partner or donor), insemination of the oocytes is performed. The resulting embryos are monitored daily in the lab and classified based on morphology and division quality.</li>

<li><strong>STEP FIVE:</strong> Embryo transfer. Once embryos are obtained, they are transferred into the uterus. This may occur in the same cycle or, in some cases, the embryos are cryopreserved and transferred in a later cycle.</li>

<li><strong>STEP SIX:</strong> Pregnancy test. A quantitative blood pregnancy test (BHCG) is performed.</li>

<li><strong>STEP SEVEN:</strong> Vitrification of remaining embryos. Embryos not used in the transfer are cryopreserved (vitrified) in the lab for potential use in future cycles.</li>
